Vital Statistics

The Office of Vital Statistics issues certificates for all Ohio births and for deaths that occurred in Madison County from 1908 to the present. We can supply forms to add a Father's name to the birth record and make typographical corrections to existing birth or death records.

Instructions for requesting birth and death certificates by mail. Each certificate is $23.00.
If paying by check, make checks payable to Madison County Health Department. Send the application along with a check or money order and self-addressed stamped envelope to:
